The Home Based Primary Care (HBPC) program provides health care to eligible Veteran patients in their homes when care in the outpatient clinics is not possible or recommended. The HBPC program provides the patient needed medical follow up as well as patient and/or family supervision and training to make the care of the patient easier, to help the patient do as much as possible for himself, and/or to help the patient be as comfortable as possible.
